McIntosh finished in four minutes, 27.11 seconds, more than four seconds ahead of runner-up Katie Grimes of the United States.
Later, the 16-year-old Toronto native added a bronze medal, anchoring the 4×100-metre medley relay team.
Kylie Masse, Sophie Angus and Maggie Mac Neil joined McIntosh for the trip to the podium.
The Americans won gold, while Australia took silver.
McIntosh also won the 400 IM at last year’s world championships
